Our client forms are electronically available through the patient portal (link below).  If you would like to add a form such as a release of information, updated credit card information for, etc.  Please log into your patient profile and select the CLINICAL DOCUMENTS on the left sidebar.  From there, select the document you would like to complete from the drop-down list and click CREATE DOCUMENT.  Fill in the required fields and sign with your password.  Your document is immediately available to your provider and, if applicable, to the office staff.


If you are a NEW PATIENT, you will be given your user name and password to log in an fill out your paperwork 2 days prior to your intake appointment.  


If you have not completed the new patient forms prior to your appointment, please arrive 45-minutes early to allow time to fill them out at our office.  Your session will be shortened if you have not completed the documents required.
