Payment options

Cash Pay: 

For those without insurance or who do not wish to bill insurance the fee is $200. Session fees are due at time of service. 

Insurance

Dr. Dinehart is in-network with some Aetna and BCBS plans.  Please check with your carrier to see if Dr. Dinehart is an in-network provider.  Having insurance does not guarantee payment for your sessions. The patient is financially responsible for all sessions.  Session fees are due at the time of service.   He does not accept Medicaid (AHCCCS) or Medicare.  

Dr. Dinehart does not offer EAP billing. 

Bishop Pay

Please speak with your bishop about authorizing sessions paid for by the bishop.   This must be arranged, with all paperwork completed, BEFORE your session in order to be billable to them.

Late Cancel/No-show Policy

Late cancels or no-shows are subject to a $200 fee if not cancelled at least 24 BUSINESS hours before your appointment.  Please contact the office directly, at 480-988-5003, for all cancellations and reschedules. 

Accepted Payment Methods

Cash, Debit Cards, all major Credit Cards, Flexible Spending Accounts (FSA), and Health Savings Accounts (HSA).  All credit cards must be present in order to be used for payment.
